Who We Serve:
Anyone in Windsor-Essex who does not currently have a family doctor or a nurse practitioner.
Who You’ll See:
The CMHA Health Centre has an integrated health team of nurse practitioners, physicians, registered nurses, physiotherapy, dietitian, massage, foot care, and preventive care.
Hours of Operation
Monday, Wednesday, Thursday: 8:30am – 7:00pm
Tuesday and Friday: 8:30am – 4:30pm

Dietitian Services
The CMHA Dietitian provides individual nutritional counselling to improve your health and well-being. Group programs are also offered with a focus on creating a healthy lifestyle.

Locally-Driven Windsor Essex
Locally Driven Population Health Models (LDPHMs) in Ontario are community-based, equity-promoting initiatives that aim to address the unique health and well-being needs of local populations – especially those facing barriers due to social determinants of health.
In Windsor-Essex, the CMHA-WECB acts as the backbone organization bringing together local community partners to co-design and deliver holistic services such as primary care, mental health support, chronic disease prevention and health literacy outreach.

A variety of clinical treatment interventions to registered patients provided on an individual or group basis.
Therapy Seminars Overview
Week One – Welcome to Therapy
Objective: After this session clients will be able to:
- Orient clients to therapy
- Identify difference between coaching, psychotherapy, and psychiatry
- Understand expectations for therapy
- Know what a SMART goal is
- Understand why goals are important
- Understand importance of activities of daily living and coping skills
- Understand how the physical, psychological, social, and spiritual are connected
- Understand how physical issues can present as psychological concerns
- Understand the importance of sleep hygiene
- Introduction to grounding and mindfulness skills
- Understand basic neurological relationship of the brain and nervous system
Outline:
- Introduction – Instruct online participants to check in at beginning, middle and end via chat

Preventive Care Specialists
Our Preventive Care Specialist focuses on the whole person and understands that health does not exist in isolation. We understand that your experiences, relationships, environments and daily lives are powerful factors that can positively or negatively impact your health.
In collaboration with our Preventive Care Specialist, you will have the opportunity to develop a personalized preventive care plan to optimize your health goals. This includes check-in conversations to celebrate your progress, discussing any adjustments that you may require in your preventive care plan, and provide you with support so you are able to identify and address any barriers early.
What Can You Expect with the Preventive Care Program
Participants can receive any of following services and supports based on their individual needs:

Accessing Primary Care
You will be provided with information about health teams and physicians accepting new patients in your area. Our Preventive Care Specialist can also support you in completing any needed referral packages to connect you with your chosen health care provider.

Cancer Care
The Preventive Care Program offers education to support you in reducing your risk for cancer. Our Preventive Care Specialist uses interactive screening tools to assess presenting risk for breast, cervical, colon or lung cancer and will support you in accessing next step screening if required. You will also be provided with strategies to create beneficial habits to adopt a healthier lifestyle.

Prediabetes/Type 2 Diabetes Support
Education and the completion of screenings can be provided to you with personalized information, so you have a better understanding of those factors that may increase your risk of prediabetes or type 2 diabetes. Our Preventive Care Specialist can support referrals to dietitian services, foot care services, retinol screening, and nutrition groups available in your area.

Chronic Disease Management
Preventive Care can occur in both a one-to-one or group setting, providing you with education focused on chronic disease management.
Our Preventive Care Specialist has the tools to complete individualized risk assessments and will work with you to support you in reducing risks by focussing on Tobacco, Alcohol and/or Marijuana use, while discussing physical activity options and providing education regarding food and nutrition choices that support your overall health.

System Navigation
Preventive Care also provides individualized service navigation with a focus on food insecurity, dental care, social services support and mental health services available to you in the community.

Low Back Pain Program
Treatment plans developed to address low back pain for patients of the CMHA Health Centre which may include:
- Physiotherapy
- Massage Therapy
- Chiropractic

Foot Care Services
An advanced foot care nurse provides specialized foot care for patients of the CMHA Health Centre.

Respiratory Therapy
Assessment and treatment of registered patients with both acute and chronic dysfunction of the cardiopulmonary system.

STOP Program (Nicotine Replacement Therapy)
Smoking cessation treatment and counselling support to help individuals with goals to reduce/quit their nicotine intake. Nicotine Replacement Therapy is only available to rostered clients of CMHA Health Centre.
