CMHA Health Centre offers primary care services for the health needs of individuals and families. The Centre further operates under the collaborative care model where the interdisciplinary team of family doctors, nurse practitioners, and other health care professionals including RPN, dietitian, therapist, health promoter, and chiropodist, take a holistic approach to patient care by addressing both the primary and the mental health needs of the population we serve. The team assesses the needs of the clients served, and creates and implements treatment plans to effectively address the identified needs. Referrals for on-going primary and mental health support are made according to the treatment needs of the patient.
The clinical team offers care using best practice guidelines and referral processes. Target Population: Families, babies, men, women, children, older adults’ gay-lesbian/transgendered, people with serious mental illness.
Eligibility
Individuals with no family doctor and live within the identified catchment area bounded by Crawford Ave, Tecumseh Rd, Central Ave, Riverside Drive and/or have a diagnosis of severe mental illness living anywhere in Windsor and Essex County without a primary care provider
How To Access The Service
Complete a new patient referral form and submit to Health Centre secretarial staff located at 1400 Windsor Avenue, Windsor, Ontario N8X3L9
